Medicare Part D - Creditable Coverage Letter - Due to beneficiaries 11-14-07

is the 2008 Medicare Prescription Drug Coverage (Part D) Creditable
Coverage letter that must be mailed to each IHS beneficiary who
is Medicare eligible and served by your site. Please forward this
e-mail the appropriate individual or department at your site that
will be mailing these letters to Medicare beneficiaries. The letter
will need to be customized with the beneficiary’s name and
address and the contact information from your site and will need
to be mailed out to be received by the beneficiary no later than
November 14, 2007.
In November 2007, your site may be contacted by Medicare beneficiaries
who are IHS eligible requesting a Medicare "Creditable Coverage"
letter. These individuals may be Tribal members that do not normally
receive health services or medications at your site. They may
need a "Creditable Coverage" letter to avoid paying
a premium penalty when enrolling in a Medicare Part D prescription
drug plan. Your site will need to establish a process to determine
if these individuals are eligible IHS beneficiaries and if they
are, your site can be provided them with a "Creditable Coverage"
letter.
